Care Management Services
Care management services assist families when a senior has more acute - in depth needs, when family members live out of state, or the senior has no family support.
Care Management Services include, but are not limited to, the following:
- In-depth, professional assessment of the senior's functioning level plus physical, social and mental health needs.
- Family counseling and recommendations. Acquiring, coordinating, and monitoring medical services.
- Advocacy during hospitilization and with medical providers.
- Monitoring home visits and escort to medical appointments.
- Finding alternative housing and care.
- Health Care Power of Attorney.
- Guardianship services (provided by non-profit 501C).
