Wisdom seeks dynamic individuals to join our growing geriatric care management team. This opportunity provides a flexible schedule, meaningful relationships with clients, their families and associates, while providing creative professional services. Must demonstrate flexibility, autonomy and enjoy traveling to client’s homes in Chicago and suburbs. MSW or RN preferred; BSW with experience considered. At least three years of related elderly, preferably care management experience.
